First Visit

1. New Patient Forms

First, when you arrive to our chiropractic office in Sarasota, we’ll ask you to fill out our new patient intake forms, if you haven’t already filled them out prior to your appointment. This paperwork provides us basic information about you, as well as your medical history and a description of your condition. You can fill these forms out ahead of time by clicking on the link in the text message or email we sent to you.

2. Chiropractor Consultation

This initial visit is designed for Dr. Hornberger to understand exactly where you are at with your health and provide a roadmap to get you feeling your absolute best.

3. Chiropractic Exam

At this time if you are in extreme acute pain, we may endeavor to get you treated immediately after your examination to get you as much immediate relief as possible.

4. Report of Findings

After the exam is completed and analyzed, on your next appointment Dr. Hornberger will give you a “report of findings” that details everything discovered during the exam. After reviewing your health history, goals, and examining your spine and X-rays (if medically necessary), Dr. Hornberger will discuss recommendations for your treatment in order to help get you to optimal health.

At the end of your second appointment (report of findings), you can choose to experience your first chiropractic adjustment. This may include spinal adjustments, intersegmental traction, and/or spinal decompression, or other procedures that you have agreed to receive based on your needs. Treatment is interactive, so you can express concerns about the different styles of treatment anytime.

6. Custom Wellness Plan

After your first treatment, our team will determine if ongoing wellness and care will make sense for your health. These plans are designed to help maintain optimal health for your spine and your body.
